This is an exciting time as we make changes to the way we deliver treatment to enhance the care for our patients who are living with and beyond cancer through direct on-site support, which will enhance patient experience, promote continuity, strengthen communication, and enable timely assessment and review of individual needs.


The postholder will work as an autonomous practitioner and provide direct support to patients receiving NSO outpatient care away from their usual, local hospital, identifying and addressing any supportive care needs.


The team is based at Weston Park Cancer Centre, but you will be expected to work flexibly across South Yorkshire and Bassetlaw; therefore, you must be a car driver/owner with a valid UK driving license. The successful candidate will require substantial post-registration experience and must demonstrate experience working with patients living with and beyond cancer.

Main duties of the job

  1. Work as an autonomous practitioner providing direct support to patients receiving NSO outpatient care away from their usual, local hospital.

  2. Support patients during their NSO outpatient appointments, identifying and addressing supportive care needs, and communicating these to the Trust CNS team for seamless care.

  3. Provide input and care to patients from each of the four Trusts outside the tertiary centre (Barnsley, Rotherham, Doncaster, and Chesterfield) and serve as a regular contact point, enabling greater continuity.

  4. Assess and respond to complex patient needs, referring site-specific issues to the relevant CNS team.

  5. Focus on ensuring patients receive high-quality, personalized care, and support, helping to shape and strengthen a cross-system support framework involving key stakeholders.

  6. Address known inequalities across NSO pathways and participate in remedial actions.

  7. Improve NSO pathway timeliness and coordination to promote positive patient experiences and compliance with Cancer Waiting Times standards.

This vacancy is advertised on a fixed-term contract basis for 12 months due to external funding. The contract's duration will be reviewed based on funding status and organizational needs.


Secondments must be approved by your current line manager prior to application.

The working hours are predominantly Monday to Friday, with occasional late shifts and weekends.

Person Specification
Experience

  • Substantial post-registered experience

  • Experience in cancer care management and knowledge of disease processes

  • Advanced analytical and clinical reasoning skills

  • Evidence of autonomous practice

  • Excellent organizational and communication skills

  • Flexible approach to service development

  • Ability to manage change effectively

  • Academic and managerial achievement evidence

  • Ability to work independently and within multidisciplinary teams

  • Experience working with GPs and CCGs

Education and Qualifications

  • Registered General Nurse

  • Nursing Degree

  • Advanced communication certificate

  • Teaching certificate

  • Post-registration academic development evidence

  • Leadership and HR management experience or willingness to undertake Masters-level study

  • APACS

  • Non-medical Prescribing
