This post is for 14 hours 45 minutes per week
The post holder is an experienced nurse who,acting within their professional boundaries, will provide care for the patientfrom initial history taking, clinical assessment, diagnosis, treatment, andevaluation of care. They will demonstrate safe, clinical decision-making andexpert care, including assessment and diagnostic skills, for patients within thegeneral practice
Thepost holder will demonstrate critical thinking in the clinical decision-makingprocess. They will work collaboratively with the PSC practices to meet theneeds of patients, supporting the delivery of policy and procedures, andproviding clinical leadership as required
Inorder to work at this level NMC requirement for advanced practice must be met. Tobe considered for this exciting role you must have: a minor illnessqualification and a Non Medical Prescribing qualification (V300)
The work to be undertaken, could be as partof a group wide triage team or within one of our seven practices
Main duties of the job Comprehensively assess, review and evaluatethe needs of patients with undifferentiated healthcare needs
Undertakeclinical assessment and providing the necessary treatment
Requestdiagnostic tests only where clinically indicated in line with organisational,local and national guidelines and where appropriate
Developa plan of care in conjunction with the patient
Toutilise effective communication to aid onward referrals to specialists asappropriate
Towork in an autonomous manner without direct supervision whilst recognisinglimitation of the practitioner role
Tocarry out enhanced clinical skills as required
Tokeep accurate and immediate records of all interventions fully utilisingsoftware systems in place
Toencourage clients to carry out self-care as appropriate

Job responsibilities Clinical Responsibilities
Comprehensivelyassess, review and evaluate the needs of patients with undifferentiatedhealthcare needs to improve their physical, psychological and mental healthwellbeing to promote self-care and reduce the need for hospitalisation. Including the needs of Children from the dateof their first birthday
Undertakingclinical assessment and providing the necessary treatment including the use of IndependentPrescribing in line with local/national policies, procedures and guidelines.This may be face to face or via other remote communication methods (such asTelephone triage)
Requestdiagnostic tests only where clinically indicated in line with organisational,local and national guidelines and were appropriate
Developa plan of care in conjunction with the patient, with the application ofclinical knowledge and, analysis of symptoms and data, identification of riskfactors associated with those conditions, recognition of early signs of acuteillness, involving the patients and the carers in the care plan and ensuring itis understood, documenting safety netting plans and processes
Toutilise effective communication to aid onward referrals to specialists asappropriate. Facilitating a coordinated primary healthcare response to service usersneeds
Towork in an autonomous manner without direct supervision whilst recognisinglimitation of the practitioner role
Tocarry out enhanced clinical skills as required

Person Specification Experience Experience of working at an advanced level with patients presenting with undifferentiated illness some of which should be in Primary care
Knowledge and experience of multi-agency working
Experience of working with an MDT with the ability to motivate others

Knowledge and Skills In depth knowledge of contemporary nursing/clinical issues, legislation relating to nursing/clinical practice, and relevant Standards and Codes (eg NMC/HCPC)
Understanding of the principles and practice of clinical governance
Familiarity with accredited clinical systems for recording patient data e.g. SystmOne
IT skills, including use of MS Word and Excel
Demonstrates effective communication and interpersonal skills
Use of a range of communication techniques
Able to use internet as an information resource, including searching databases
Demonstrable/proven leadership skills
Qualifications Registered General Nurse, with current NMC registration
