Work as an important member of the Multidisciplinary Team within Sheffield Teaching Hospitals NHS Foundation Trust providing specialist nursing advice and support to patients and their family, throughout the care pathway



The post holder will utilise skills of clinical expertise, education, and management to ensure a seamless service for patients, carers, and staff providing advice, support, and information



You will work closely with the multidisciplinary team in hospital and across the community to ensure a coordinated quality service and will be actively involved in education, audit, and research



The post holder will engage and lead discussions in multidisciplinary team meetings and ensure a high quality, holistic approach to the management of the patient’s pathway is maintained

This vacancy is advertised on a fixed term contract basis for a period of 6 months due to the post being recruited as part of a limited term project within the Trust. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the requirement for the post to be maintained as a part of the project’s implementation and the timescales associated with this
