Shifts between: 08:00 - 22:00
Shift patterns: 08:00 - 16:30 or 10:00 - 18:30
Pay rate: £17.18 per hour
1 hour paid lunch
We are seeking a dedicated Support Assessor to join our team. This role involves assessing, developing, and contributing to the monitoring and review of individual reablement programmes within the provider service. The aim is to meet the identified needs of service users, including those with complex and changing needs, to promote independence, choice, control, and social inclusion.
Day-to-Day Responsibilities:
- Carry out service-specific assessments to identify individual needs within the provider service remit.
- Develop individual programmes to meet the needs of all service users, including those with complex and changing needs.
- Plan and organise work to meet personal and service targets and deadlines.
- Work in partnership with other team members, services, and organisations as required.
- Monitor and review individual programmes with service users, carers, staff, and other professionals using agreed systems of communication.
- Follow appropriate systems, processes, and communications to ensure services are set up and delivered on time.
- Record user information, maintain and update files, and prepare and produce reports.
- Participate in training, supervision, and appraisal to ensure the delivery of a quality service.
- Attend staff, user, and carer consultation meetings to assist in the development and planning of future services.
- Work within agreed health and safety procedures and undertake risk assessments as needed.
- Work flexibly across the city within Adult Provider Services, including evenings and weekends as determined by service needs.
Required Skills & Qualifications:
- Understanding of the social care needs of older people and adults.
- Ability to translate individual needs into reablement programmes that respect the preferences, rights, and choices of the individual.
- Experience in social care provision, particularly with older people or adults with complex needs.
- Effective team working and multi-disciplinary collaboration skills.
- Proficiency in IT applications to support service delivery.
MUST HAVE a Driving license and access to a vehicle
